Closes https://github.com/tendermint/tendermint/issues/3857

Moves `lib/` folder to `jsonrpc/`.

Renames:

**packages**

`rpc` package -> `jsonrpc` package
`rpcclient` package -> `client` package
`rpcserver` package -> `server` package

**structs and interfaces**

- unexpose `ResponseWriterWrapper`

Refs #4319

No need to duplicate information in this case. It a) requires extra
efforts to keep both in sync b) nobody reads godoc documentation
anyways.

* limit number of /subscribe clients and queries per client

Add the following config variables (under [rpc] section):
  * max_subscription_clients
  * max_subscriptions_per_client
  * timeout_broadcast_tx_commit
