This changes adds an `MempoolError` field to the `ResponseCheckTx`. This will allow clients to understand that their transaction was rejected from the mempool despite passing the ABCI check.
This change also updates the code to make use of early returns to prevent highly nested code blocks. Namely, it returns when the type assertion fails at the beginning of the method, instead of wrapping the entire method in a large if statement. This has a somewhat large effect on the diff as rendered by github.

Adds a genesis parameter `initial_height` which specifies the initial block height, as well as ABCI `RequestInitChain.InitialHeight` to pass it to the ABCI application, and `State.InitialHeight` to keep track of the initial height throughout the code. Fixes#2543, based on [RFC-002](https://github.com/tendermint/spec/pull/119). Spec changes in https://github.com/tendermint/spec/pull/135.
Reorganizes the Protobuf schemas. It is mostly bikeshedding, so if something is contentious or causes a lot of extra work then I'm fine with reverting. Some Protobuf and Go import paths will change.

## Description
This PR removes simple prefix from all types in the crypto/merkle directory.
The two proto types `Proof` & `ProofOp` have been moved to the `proto/crypto/merkle` directory.
proto messge `Proof` was renamed to `ProofOps` and `SimpleProof` message to `Proof`.

Fix some linter issues to conform with the Protobuf style guide.
The state sync enum changes are ok to break since it's not released yet. Personally I find the uppercase kind of ugly, but that's what the guide says. Couldn't find a way to generate camel case in Go, short of specifying custom names for each and every enum variant.
Another option would be to simply disable the enum case lint.

This pull request is prefixing all the types in proto to avoid conflict.
When a go application is using Tendermint as a library and also define similar types in gogo proto some conflicts might occur (as types is a common package in go).
By prefixing the types with tendermint, this highly reduces the risk of conflicts.
BREAKING CHANGE.
This modification breaks the ABCI Application endpoint.
What was accessible before with `/types.ABCIApplication/Flush` is now accessible with `/tendermint.abci.types.ABCIApplication/Flush`.

As per #2127, this refactors the RequestCheckTx ProtoBuf struct to allow for a flag indicating whether a query is a recheck or not (and allows for possible future, more nuanced states).
In order to pass this extended information through to the ABCI app, the proxy.AppConnMempool (and, for consistency, the proxy.AppConnConsensus) interface seems to need to be refactored along with abcicli.Client.
And, as per this comment, I've made the following modification to the protobuf definition for the RequestCheckTx structure:

This PR introduces a fundamental breaking change to the structure of ABCI response and tx tags and the way they're processed. Namely, the SDK can support more complex and aggregated events for distribution and slashing. In addition, block responses can include duplicate keys in events.
Implement new Event type. An event has a type and a list of KV pairs (ie. list-of-lists). Typical events may look like:

"sender": [{"address": "...", "balance": "100uatom"}]
The events are indexed by {even.type}.{even.attribute[i].key}/.... In this case a client would subscribe or query for rewards.recipient='...'
ABCI response types and related types now include Events []Event instead of Tags []cmn.KVPair.
PubSub logic now publishes/matches against map[string][]string instead of map[string]string to support duplicate keys in response events (from #1385). A match is successful if the value is found in the slice of strings.
